I, through myself or my agents, hereby voluntarily upload any information, documents, images, lab results, testing, and any other medical documents to TAPMI and authorize agent, employees, and medical experts to use or disclose my health information, at my request, during the term of this Authorization.

You are responsible for updating and providing accurate documents and any and all medical test results into your medical archive.

Any and all information obtained as a result of uploading documents and test results to your medical archive does not render a medical opinion on behalf of  TAPMI.

It is your responsibility to input all accurate information and lab test results, images, and numbers into your medical archive.

You understand that reference values may vary slightly among laboratories. Our system uses the reference values of leading medical institutions as a base and can vary.

Charts, tables and informative links do not provide medical advice. TAPMI online medical archive service (TAPMI) is not an alternative for a face-to-face doctor consultation.

Effective organization of the uploaded files is your responsibility.

You have the right to authorize a third party to create, upload and update documents on your behalf to your medical archive. You have the right to rescind the authorization at any time.

The medical archive (TAPMI) service provided by TAPMI  is a completely separate and independent service. If you would like to use the TAPMI service (medical opinion) to have a doctor give a second opinion based on the documents you selected for him/her to review from your TAPMI archive, you are required to make the payment for fees associated with the medical opinion (TAPMI) service.

All documents and files uploaded to your medical archive are private and confidential.

You understand that it is your responsibility to assign any documents and results from your medical archive to your TAPMI case.

You understand that the doctors can only view and assess the documents that you assign to your TAPMI case.

TAPMI  will keep all medical archive data in your medical archive only for a limited period. All documents and data will be retained on a continued renewal basis. If you fail to renew with TAPMI, all documents and data in your archive will only be retained for three (3) months upon the expiration of your membership. Documents and data in your medical archive will not be available after three (3) months after the expiration of membership and failure to renew.

All documents and data in your medical archive can only be provided to doctors and providers associated with TAPMI. TAPMI will not provide any documents and data in your medical archive to any person, party or facility not associated with TAPMI NETWORK.

TAPMI will not provide copies of any documents and dates in your medical archive to you or any third party not associated with TAPMI NETWORK.

Any use or attempted use of TAPMI (i) for any unlawful, unauthorized, fraudulent or malicious purpose, or (ii) that could damage, disable, overburden, or impair any server, or the network(s) connected to any server, or (iii) interfere with any other party's use and enjoyment of the Website, or (iv) to gain unauthorized access to any other accounts, computer systems or networks connected to any server or systems through hacking, password mining or any other means, or (v) to access systems, data or information not intended by TAPMI  to be made accessible to a user, or (vi) attempt to obtain any materials or information through any means not intentionally made available by TAPMI, or (vii) any use other than the business purpose for which it was intended, is prohibited.

TAPMI NETWORK does not share any of your medical information with third parties without your permission and knowledge. You understand that there may be extreme circumstances in which TAPMI may be legally required to share the information based on subpoena or court order.

You understand that TAPMI is not responsible for any breach of its security systems that may affect you.

A TAPMI user can return the Products, if only it has not been Used and Registered to an Account and/or a Person(s).

TAPMI service fee is Non-Refundable. Under Extreme Circumstances the Charges will be Refunded Depending on the Nature of the Situation on a Discretionary Basis.
